Dental implants have a 95–98% success rate at 10 years — one of the highest in all of dentistry. But 2–5% failure means some patients will experience implant loss. The primary risk factors are smoking (2x higher failure rate), uncontrolled diabetes, bruxism (grinding), poor oral hygiene, and insufficient bone quality. Early vs Late Failure

Early failure (within the first 3–6 months, before osseointegration is complete) accounts for most implant failures. The implant never fully integrates with the bone and becomes mobile. Causes include surgical trauma, infection, premature loading (putting too much force on the implant before it has healed), overheating during placement, or poor bone quality that was not adequately assessed pre-operatively.

Late failure (after successful osseointegration, sometimes years later) is less common and typically results from peri-implantitis (gum disease around the implant), mechanical overload from bruxism, cement or screw loosening causing chronic inflammation, or trauma. Late failure is usually slower and more predictable, often giving the patient and dentist time to intervene before complete implant loss.
